Refactor Admin HTML Layout to be Consistent with View

The HTML page layout structs are very similar but the admin app could be updated to use a content table structure like the view apps. The benefit is that the associated CSS could be used straight up between the 2 apps. As of 7/2011 most of the code can be used but they do differ.

TEST: Make sure admin and view apps look okay on various computers, browsers etc.

TESTING: Look for issues with the page look especially the navigation, header, footer areas of the admin app and view app.

Upon testing done on 6/5, Kim noticed 1) an issue with the date format in the footer of the query builder page and 2) the "Data Chart" line no longer appears in the query criteria selection on the query report page (not sure if that was intentional).
