63                                Analysis of public health data by demographic characteristics is essential
64                                to the reduction and elimination of health disparities. The
65                                <a href="http://www.gpo.gov/fdsys/pkg/PLAW-106publ525/pdf/PLAW-106publ525.pdf">
66                                Minority Health and Health Disparities Research and Education Act of 2000</a> 
67                                describes health disparities as differences in "the overall rate of disease
68                                incidence, prevalence, morbidity, mortality, or survival rates in the population
69                                as compared to the health status of the general population." The definition can
70                                be applied to any demographic group, not just racial/ethnic minorities. Analysis
71                                by demographic characteristics also shows at what age certain diseases
72                                and conditions typically appear.<br/>

74                                Health equity is the principle to pursue the highest possible standard of health
75                                for all while focusing on those with the greatest obstacles. Social determinants
76                                have a large impact on disparities and health equity. In order to improve health
77                                outcomes for those with disparities, social determinants often need to be 
78                                targeted for intervention and prevention efforts. Analysis of what social
79                                determinants affect which populations in what areas helps inform programs where
80                                to focus efforts. <a href="http://www.countyhealthrankings.org/what-is-health">
81                                County Health Rankings</a> estimates that social, economic, and
82                                physical environment factors account for 50% of health factors.<br/>
83                                <br/>
84                                Interventions with the largest impacts on health outcomes are population-based
85                                and focus on social determinants. CDC currently has an effort to target community
86                                approaches that focus on "social determinants of health" and "changing the
87                                context to make the healthy choice the easy choice." For information on the
